29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: A.) Crossett, Inc. - On or about 3/1/13, employees were performing lockout/tagout on machinery without specific procedures. The machinery that servicing and maintenance were being performed on included, but was not limited to, the waste oil heater, tanker trucks, and the air compressor. Servicing and maintenance activities included but were not limited to, repairs and preventative maintenance. The procedures were deficient in that they were missing the magnitudes of energy, steps to release from lockout, notifying affected employees, specific instructions on how to dissipate stored energy or how to isolate it, and specific means to verify deenergization.
