29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Eleven employees were exposed to an approximately 13 foot fall due to the use of an extension ladder which did not have the ladder's side rails extend at least 3 feet about the upper landing surface being accessed.

29 CFR 1926.20(b)(2)    The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):                                                                       Eleven employees were working on an approximately 13 foot high 8 in 12 pitch roof  without fall protection to the lower level and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.

29 CFR 1926.501(b)(11)     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:     Eleven employees working on an approximately 8 in 12 pitch roof which was approximately 11 feet from a lower level was exposed to fall hazards while conducting work without guardrail systems, safety net systems, or personal fall arrest systems.
