Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment, which was free from recognized hazards that was causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn and explosion hazards: Employees, working in the area of the natural gas heaters, were exposed to burn and explosion hazards in that the HeatStar HS125NG area heaters were provided with natural gas through the use of Grade R Acetylene Only flexible hose and the area heaters were used as permanent area heaters for general industry instead of the manufacturers intended use as a heater for buildings under construction, alteration, or repair. Among other methods, the following is a feasible and acceptable method to correct this hazard. NFPA 54 / ANSI Z223.1 2012 Section 9.1.1 which states Appliances, equipment, and accessories shall be approved. Remove the HeatStar HS125NG area heater from service in the facility. Discontinue use of incompatible hose to supply fuel-gas.

29 CFR 1910.253(e)(5)(i): Hose for oxy-fuel gas service did not comply with the Specification for Rubber Welding Hose, 1958, Compressed Gas Association and Rubber Manufacturers Association: Employees performing fuel gas and oxygen cutting were exposed to burn and explosion hazards in that the Grade R Acetylene Only hose was not intended for use with the facility natural gas supply and the employer did not ensure the hose for oxygen and fuel gas service complied with the Specification for Rubber Welding Hose, 1958, Compressed Gas Association and Rubber Manufacturers Association.
