29 CFR 1910.266(d)(1)(vii)(A):     The employer did not provide, at no cost to the employee, and ensure that each employee wear eye protection meeting the requirements of subpart I of Part 1910 where there was potential for eye injury due to falling or flying objects:    (a)  Felling Area:  The employer did not ensure that the skidder operator hooking up trees to the cable skidder wore eye protection, for protecting the eyes from being struck by a branch, limb, or sapling, as determined on March 5, 2013.    (b)  Felling Area:  The employer did not ensure that the timber cutter wore eye protection while operating a Husqvarna 372XP chainsaw to fell trees, for protecting the eyes from wood chips during chainsaw operation, as determined on March 5, 2013.      Mill Creek Builders Supply Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.266)(d)(1)(vii)(A)), which was contained in OSHA inspection number 313372062, citation number 1, item number 1 and was affirmed as a final order on 03/03/2010, with respect to a workplace located at Kelly Mountain Tract, Elkins, WV  26230.      Abatement Certification and Documentation Required for this Item.

The employee's CPR certification expired on May 25, 2011, as determined on March 5, 2013.    (b)  Worksite:  The CPR training certification for the grapple skidder operator was not current.  The employee's CPR certification expired on May 25, 2011, as determined on March 5, 2013.     (c)  Worksite:  The CPR and First Aid training certification for the dozer operator was current.  The employer could not show the current status of the training certifications, as determined on March 6, 2013. 29 CFR 1910.266(d)(2)(ii):   Each first aid kit did not contain the items listed in Appendix A at all times:  (a)  John Deere Dozer:  The first aid kit on the John Deere 650J dozer did not contain at a minimum all items listed in Appendix A of 29 CFR 1910.266 in that it did not contain; gauze pads at least 4 x 4 inches, two large gauze pads at least 8 x 10 inches, roller bandages, scissors, tweezers, triangle bandage, blanket, resuscitation equipment, and two elastic wraps, as determined on March 5, 2013.  (b)  John Deere Cable Skidder:  The first aid kit on the John Deere 540 cable skidder did not contain at a minimum all items listed in Appendix A of 29 CFR 1910.266 in that it did not contain; gauze pads at least 4 x 4 inches, two large gauze pads at least 8 x 10 inches, roller bandages, scissors, tweezers, triangle bandage, blanket, resuscitation equipment, and two elastic wraps, as determined on March 5, 2013.  (c)  Prentice Loader:  The first aid kit on the Prentice 280 loader did not contain at a minimum all items listed in Appendix A of 29 CFR 1910.266 in that it did not contain; resuscitation equipment, as determined on March 5, 2013.  (d)  John Deere Grapple Skidder:  The first aid kit on the John Deere 548 G3 grapple skidder did not contain at a minimum all items listed in Appendix A of 29 CFR 1910.266 in that it did not contain; two large gauze pads at least 8 x 10 inches, as determined on March 5, 2013.
