29 CFR 1926.251(d)(2)(v): Knots were used in lieu of splices: a) Speelman Electric, Inc. on or about March 7, 2013, at the City of Avons water tower, near the intersection of Detroit Rd. and Center Rd. Avon, OH: The employer failed to ensure a synthetic rope sling used to hoist approximately 160 ft. of fiber optic cable was adequately spliced. A knot was utilized to create hoisting loop on the aforementioned rope sling, degrading the lifting capabilities. Employees working under the hoisted load guiding the cable were exposed to struck-by injuries caused by fallen equipment/material.

29 CFR 1926.1425(b): While the operator was not moving the suspended load, employees were within the fall zone, except those employees engaged in; (b)(1): hooking, unhooking or guiding a load; (b)(2) initial attachment of the load to a component or structure or (b)(3) operating a concrete hopper or bucket. a) Speelman Electric, Inc. on or about March 7, 2013, at the City of Avons water tower, near the intersection of Detroit Rd. and Center Rd. Avon, OH: The employer failed to ensure employees were kept out of the fall zone of a cranes suspended load. At ground level, employees were preparing the ends of fiber optic cables directly below a suspended personnel platform in which an employee was working above. Employees working in the fall zone were exposed to stuck-by and crushed-by hazards.

29 CFR 1926.1431(g)(1)(i)(B): Hooks used in the connection between the hoist line and the personnel platform (including hooks on overhaul ball assemblies, lower load blocks, bridle legs, or other attachment assemblies or components) were not closed and locked when attached. a) Speelman Electric, Inc. on or about March 7, 2013, at the City of Avons water tower, near the intersection of Detroit Rd. and Center Rd. Avon, OH: The employer failed to ensure the throat latch of the cranes load line hook was locked prior to hoisting employees in a personnel platform. In the event tension was lost in the load line, the hooks unsecured latch provides inadequate defense in retaining the platforms master link.
