Avon, OH: The employer failed to ensure a synthetic rope sling used to hoist approximately 160 ft. of fiber optic cable was adequately spliced. A knot was utilized to create hoisting loop on the aforementioned rope sling, degrading the lifting capabilities. Employees working under the hoisted load guiding the cable were exposed to struck-by injuries caused by fallen equipment/material.
