OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to combustible fiberglass/resin dust explosion, deflagration, or other fire hazards while working at or near dust collection system which was not adequately designed to prevent or minimize employee exposure in the event of a deflagration or other uncontrolled fire event:   On or about March 6, 2013   a) The outside Dust Control media-type and cyclone dust collectors handling combustible fiberglass/resin dust lacked a means of a deflagration propagation isolation to prevent the potential propagation of a deflagration, originating from the outside dust collectors, back into the workplace.    Among other methods, one feasible means of abatement to correct this hazard would be to comply with The National Fire Protection Association (NFPA) 654 "Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids, 2013 ed.", section 7.1.6 and 7.1.7 by installing a listed deflagration isolation device in a safe location/manner downstream of the process and upstream of the outdoor dust collectors in accordance with NFPA 69, "Standard on Explosion Prevention Systems."  Installing explosion protection systems on the outdoor dust collectors, in addition to an isolation device, is an additional recognized practice which may further reduce the hazard.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  A lockout/tagout program was not developed for the Pulltrusion lines including saws and other equipment.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  Fork truck drivers were not evaluated every three years.
