29 CFR 1910.132(d)(1): Hazard assessment and equipment selection. The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).   a. Danite Holdings LTD, 1640 Harmon Avenue, Columbus, Ohio workplace: as of and prior to March 7, 2013, the employer had not performed and documented a personal protective equipment hazard assessment, for hazards including but not limited to those associated with paint mixing operations. Paint mixing operations involving skin contact and absorption hazards associated with hazardous chemicals such as "di-isocyanates" which include hexamethylene-di-isocyanate, a hazardous ingredient of "43270SP Catalyst."

Danite Holdings LTD, 1640 Harmon Avenue, Columbus, Ohio workplace: as of and prior to March 7, 2013, the employer had not established machine specific procedures for the control of hazardous energy (Lockout/Tagout) for employees performing specific duties covered by this part; such as, but not limited to set-up on the Chamfer and blade changes/adjustments on the aluminum cutter/saw machine in the production area.

29 CFR 1910.303(b)(1)(iv): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: Electrical insulation;  a. Danite Holdings LTD, 1640 Harmon Avenue, Columbus, Ohio workplace: as of and prior to March 7, 2013, the vertical drop electrical power supply cord for the trim cap area work station had damaged electrical insulation. Employees in this work area have exposure to this electrical contact/shock hazard during power tool use, such as jig saw use.  b. Danite Holdings LTD, 1640 Harmon Avenue, Columbus, Ohio workplace: as of and prior to March 7, 2013, the portable subpanel box power supply wiring used for testing digital sign boards did not have proper protection by electrical insulation, enclosure or conduit for live parts. Wire nut splices used to make connections were open to contact, displacement, pull or strain and the live wires were not otherwise protected against accidental contact without insulating material or mechanical protection.
