Eco Southeast, LLC - on or about March 8, 2013, an employee, primarily operating a forklift during monitoring, was determined to have an exposure of 123.1% of the permissible daily limit (equivalent dBA of 91.5 as an 8-hour TWA).  The employee was monitored for 349 minutes, with 131 minutes included with an assumed exposure level of 0 dBA.  The employer had not developed and implemented a Hearing Conservation Program.
