29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Steel Huggers, LLC at 2791 W. 52nd Ave., Denver, CO 80223: On and before March 7, 2013, the exposing employer did not ensure that employees were protected from injury, in that two employees were engaged in the installation of a fixed ladder on a roof of an apartment building with a fall hazard of approximately 10-12 feet without the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Steel Huggers, LLC at 2791 W. 52nd Ave., Denver, CO 80223: On and before March 7, 2013, the exposing employer did not ensure that the ladder employees were using to access an upper landing surface extended 3 feet above the upper landing area. This condition exposed employees to a fall hazard of approximately 10-12 feet.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated in the safe operation of the forklifts in the workplace (a) Steel Huggers, LLC at 2791 W. 52nd Ave., Denver, CO 80223: On and before March 7, 2013, the exposing employer did not certify that the forklift operators had been trained and evaluated as required by 29 CFR 1910.178(l) in the topics as outlined in 29 CFR 1910.178(l)(3). Abatement Note: The certification shall include: (1) The name of the operator; (2) The date of the training; (3) The date of the evaluation; and, (4) The identity of the person(s) performing the training and evaluation.
