29 CFR 1910.23(a)(5): For every pit and/or trap door floor opening, while the cover was not in place, was not constantly attended by someone or was not protected on all exposing sides by removable standard railings: a) 720 Ashley Avenue, Brielle, NJ: Employees were exposed to a fall hazard when the cover for the floor opening was removed for maintenance of the equipment in the crawl space. The opening was not guarded to prevent an employee from falling into the opening to the lower level, observed on or about 2/14/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
