29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    (a) At 1503 South Hwy 69 in Nederland, Texas, on the east side of the building, on or about March 08, 2013 - Employees were exposed to fall hazards of approximately 13 feet while performing roofing work and staging materials without using fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    (a) On the east side of 1503 South Hwy 69 in Nederland, Texas, on or about March 08, 2013 - Employees were exposed to fall hazards of approximately 13 feet while using a ladder not extending at least 3 feet above the work surface while staging materials to perform roofing work.
