29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On March 7, 2013, in the Maintenance Shop of building #52 (Middle Building), employees were exposed to serious electrical hazards created by the Delta Bench Grinder model 23-660, in that the grinder had been attached to an extension cord which was not in accordance with the listing and labeling for usage with the equipment being used. Abatement certification is not required for this item.

Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
