29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation, and the operator does not have a clear and direct view of the edge. (a) On or about 03/08/13, at the Victor Salerno Center, under construction and located on the campus of St. John Fisher College at 3690 East Avenue, in Rochester, New York, an excavated opening did not have barriers or other warning systems to prevent mobile equipment such as loaders, excavators and lifts from traveling into the excavation. NO ABATEMENT CERTIFICATION REQUIRED
