29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize those hazards: (a) On or about February 14, 2013, at a job site located at 3600 Wyandotte Street in Kansas City, MO, employees performing roofing work were exposed to serious fall hazards due to not having been trained in the proper use of personal fall arrest systems. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface the ladders' side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about February 14, 2013, at a job site located at 3600 Wyandotte Street in Kansas City, MO, employees using extension ladders to access the various roof levels were exposed to potential fall hazards due to the ladders' side rails not extending at least 3 feet above each upper landing surface. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
