29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition: On or about March 12, 2013, in the facility, employees required to perform loading and unloading operations were exposed to trip and fall hazards from process hoses in and across passageways ("tracks", etc).

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder: On or about March 12, 2013, at the reactor building, employees (Operators/Vat Techs) required to access tanks/vessels were exposed to a fall hazard of approximately 8 to 16 feet above ground level due to partially unguarded catwalk platforms currently under construction.

29 CFR 1910.1450(e)(1): Where hazardous chemicals as defined by this standard are used in the workplace, the employer shall develop and carry out the provisions of a written Chemical Hygiene Plan: On or about March 12, 2013, in the facility lab, employees required to perform material testing/analysis were exposed to the hazard of contact with hazardous chemicals including acids, bases and solvents. The employer did not develop a written Chemical Hygiene Plan.
