29 CFR 1910.334(a)(2)(ii): Where there was a defect or evidence of damage that might expose an employee to injury, the defective or damaged item was not removed from service until repairs and tests necessary to render the equipment safe had been made: a) On or about March 12, and at times prior, employees were exposed to electrical hazards where on the cords for two portable fans the outer insulation was pulled back, exposing the conductors inside the cords. Abatement Note: Abatement certification is not required for this item.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) On or about March 12, 2013, and at times prior, employees were exposed to electrical hazards where a flexible cord connected to an outet box did not have adequate strain relief. The outer insulation to the cord was pulled back, exposing the conductors inside the cord. Abatement Note: Abatement certification is not required for this item.
