Electrical insulation.  (a) 293 Saints Trail NW, Brookhaven, MS - On or about March 12, 2013 employee was operating a drill connected to a flexible cord with the outer electrical insulation broken in multiple parts of the cord.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  (a) 293 Saints Trail NW, Brookhaven, MS - On or about March 12, 2013 employees were using multiple extension cords and drill with defective strain reliefs.
