29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about March 12, 2013, employees performing masonry stone installation activities from a (6) tier tubular welded frame scaffold that was not equipped with end guardrails. The employees were exposed to fall hazards of up to (21) feet to the ground.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about March 12, 2013, employees working on or near a (6) tier tubular welded frame scaffold had no additional protection from being struck by objects and/or tools falling from a height of up to (21) feet through installation of toe boards.
