29 CFR 1910.264(d)(2)(i)(a): The employer did not ensure safeguards, safety appliances, or devices attached to, or forming an integral part of any machinery were not removed or made ineffective except for the purpose of making immediate repairs or adjustments: This violation was observed on March 12, 2012 in the following instances: a) On the Chicago Edge Blanket Feeder #1, the inlet pinch sensor had been bypassed/inoperable exposing the employees to caught in hazards. b) On the Chicago Edge Blanket Feeder #2, the top safety sensors were misaligned which made them inoperable to stop the machine when tripped exposing employees to caught in hazards. c) On the Chicago Edge Blanket Feeder #2, the left panel door safety switch was inoperable exposing employees to caught in hazards. d) On the Chicago Edge Blanket Feeder #2, the inlet pinch sensor had been bypassed/inoperable exposing the employees to caught in hazards.
