29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a)Gonzalez Roofing @ 13598 Via Varra St., Broomfield, CO 80020: On and before March 8th, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected edges of a roof on a multi-family complex. Employees were not utilizing an adequate fall protection system. Employees were exposed to fall hazards of approximately 45'.

1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grab-rail, was not provided to assist the employees in mounting and dismounting the ladder (a)Gonzalez Roofing @ 13598 Via Varra St., Broomfield, CO 80020: On and before March 8th, 2013, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. Employees were using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.
