29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On or about March 12, 2013 and at times prior, Pueblo Mall LLC as the controlling/correcting employer, did not ensure that employees of Griffith Maintenance, Inc. were protected from injury in that, energized electrical outlets were not properly installed according to their listing and labeling. The electrical outlets were loose and were not secured to the electrical outlet box.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:(a)(LOCATION)(IDENTIFY EQUIPMENT AND CONDITION(S))(STATE HAZARD(S) TO EMPLOYEE(S)) (a) On or about March 12, 2013 and at times prior, Pueblo Mall LLC as the controlling/correcting employer, did not ensure that employees of Griffith Maintenance, Inc. were protected from injury in that, the flexible cords were not installed as substitute for fixed wiring. The flexible cords were used to provide power to the energized electric light fixtures inside a maintenance shed. The employees used the electric light fixtures and was exposed to electric shock.

The employees used the electric light fixtures were exposed to electric shock.
