Note.  Each opening requires both a top and mid-rail (chain).

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)     a) Yard Area - Taylor Forklift # 531: The forklift was not taken out of service for an inoperable horn, on or about 03/13/13.     b) Yard Area - Gradall Forklift # 533 Model # 534C-9:  The forklift was not taken out of service for an inoperable overhead left light lense.     c) Yard Area - Gradall Forklift # 533 Model # 534C-9:  The brake pedal did not function properly. As observed on or about 04/18/13, when the brake inching travel pedal was depressed, the pedal dropped to the floor and had to be lifted up by hand to reset the brake in its normal position.
