29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Location: 13909 Lynmar Blvd in Tampa (FL) (a) for the employees using the Komatsu forklift with a man basket attachment for changing light bulbs, installing roll power cords to the ceiling, cleaning and repairing the plastic barrier, as observed on or about March 13, 2013. The employees in the man basket attachment of the forklift were not provided and/or required to use a personal fall protection system. Employees were exposed to fall hazards up to about 20 feet.
