29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    An employee performing servicing and maintenance operations was exposed to hazards associated with the unexpected energizing or start-up of equipment to include the Accurpress 7254machines; the Fadal VMC4020A CNC Mill machines; the US Industrial shear machine; the Verson Wheelon Bladder Press machine; the Bliss Press machine; The Keith DB1 machine; and the Multi Cam CNC Router machines.  Written machine specific energy control procedures for these machines were not developed and implemented.    29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.    The Cox Machine, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 315087320, citation number 1, item number 1 and was affirmed as a final order on June 02, 2011, with respect to a workplace located at 5338 West 21st St., Suite 100, Wichita, KS 67205.
