29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: JOB SITE: 10441 Olive Boulevard, Creve Coeur, MO 63121, the employer failed to conduct frequent and regular inspections of equipment by a competent person. The hand rail attached to the steps leading to the basket of the aerial lift was broken and detached from the frame of the steps. The worker is exposed to a fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: JOB SITE: 10441 Olive Boulevard, Creve Coeur, MO 63141, at this work site, a worker was observed climbing onto the basket of a truck mounted aerial lift, to an elevated height approximately 11 feet 5 inches above ground level, without a body belt and/or lanyard attached to the basket of the areial lift. The worker was exposed to fall hazard.
