29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about March 12, 2013, at the worksite located at 710 West Main in Cabot, Arkansas, east side of building being constructed, employee working on a pitched roof engaged in pipe venting tasks did not wear fall protection and was not protected from falling to lower level. Employee was exposed to an approximately 25 feet fall from elevation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 12, 2013, at the worksite located at 710 West Main in Cabot, Arkansas, east side of building being constructed, the Werner 24 ft. extension ladder used to access and descend from the roof did not extend above the roof landing surface. This condition exposed employees to the hazard of falls from elevations.
