29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not removed from service:    a) On or about March 14, 2013, the employer failed to remove the Yale Lift Truck (model U3026; serial E187V18936Z) from service during a time in which it was in need of repair. Employees were exposed to injuries related to unsafe operating conditions in that the forklift's horn was defective.

Defects in forklift operations could potentially expose employees to struck-by and caught-between hazards.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, was not provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.  a) On or about March 14, 2013, the employer failed to ensure that employees who were provided filtering face pieces for optional use were provided with Appendix D of the respiratory protection standard. Employees were not informed of the potential hazards associated with wearing a respirator.

Employees working in the inner and outer welding areas are potentially exposed to hazardous substances found in fluxes, coatings, coverings, and filler metals in the form of lead, tin, zinc, and other metal fumes.
