OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed and/or hit by a falling load: a. 13 Brooklake Road, Florham Park, NJ The operator of a trolley boom delivery truck was exposed to being crushed and/or hit by a falling load due to the operator not making adequate inspections of the lifting system before operation. Violation observed on or about 03/13/2013.

29 CFR 1910.180(h)(3)(ix): Outriggers shall be used when the load to be handled at that particular radius exceeds the rated load without outriggers as given by the manufacturer for that crane. Where floats are used they shall be securely attached to the outriggers. Wood blocks used to support outriggers shall: a) 13 Brooklake Road Florham Park, NJ Employee(s) operating a Side-O-Matic Reciprocating Trolley Boom on unlevel surface with loose dirt. The outrigger pads used to stabalize the truck were not being supported by wood blocks or another dense surface. Violation observed on or about 03/13/2013
