29 CFR 1910.1000(a)(2):  Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million (ppm):  a)  In the mold pouring and cooling area, an Iron-Pourer employee was exposed to carbon monoxide at a level of 58 ppm as an 8-hour time weighted average (TWA) on 05/07/2013, approximately 1.2 times the permissible exposure limit. This exposure was determined by air sampling conducted over 428 minutes during one shift, with zero exposure being assumed for the unsampled 52 minutes of an 8-hour workshift.  ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.1000(e):  Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  a)  In the mold pouring and cooling area, an Iron-Pourer employee was exposed to carbon monoxide at a level of 58 ppm as an 8-hour time weighted average (TWA) on 05/07/2013, approximately 1.2 times the permissible exposure limit. This exposure was determined by air sampling conducted over 428 minutes during one shift, with zero exposure being assumed for the unsampled 52 minutes of an 8-hour workshift.  Controls in these circumstances may include, but are not limited to, the following  1) Creation of an air exhausted enclosure into/under which molds will move soon after they are poured.  2) Creation of fresh air islands, where clean air is introduced to a location where employees can be generally stationed while doing their pouring and shift work.  3) Monitor the air in this area to identify areas and times when contaminant levels are the highest, and to verify the effectiveness of controls.  4) Ensure existing exhaust ventilation fans above cooling floor are working efficiently.  5) Direct recirculated air in a manner to bring this relatively cleaner air into areas where employees are stationed, or where it can be used to direct contaminated air away from employees.  6) Retain the services of professionals in the field of foundry ventilation to design and help control air moving systems to gain the maximum effect.  ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.134(g)(2)(i):  When there was a change in the work area, conditions, or degree of employee exposure or stress that may effect respirator effectiveness, the employer did not reevaluate the continued effectiveness of the respirator:  a)  On 05/07/2013, the employer did not reevaluate the continued effectiveness of the filtering facepiece (dust mask) respirator that a Facing employee wore when exposed to respirable crystalline silica dust.  ABATEMENT CERTIFICATION REQUIRED.
