29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.      On or about March 12, 2013, inside the building on the east side, an employee engaged in electrical activities used electrically defective equipment.  The employee was exposed to electrical shock hazards with the use of:      a)  Wagner heater gun power cord missing a strain relief device to prevent pull from the terminal screws, and     b)  Extension cord utilized to energize a Milwaukee saw-zall missing a strain relief device to prevent pull from the terminal screws.
