The inadequate visual inspection is based upon the fact that an employee was allowed to enter an excavation with multiple issues affecting employee safety. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The excavation was greater than 5.2 feet in depth at a number of points. Excavations in excess of 5 feet require the use of protective systems.
