29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a. At the construction worksite, 1406 Hwy 63S, Vienna, MO, employees worked on scaffolding, with one section over three tiers high, that was not fully planked (pick boards used allowed for three boards, but two were used).

At the construction worksite, 1406 Hwy 63S, Vienna, MO, employees worked on scaffolding, with one upright leg without a base plate, and three uprights without mud sills.

29 CFR 1926.451(g)(4)(ii):     The top edge height of the guardrail on all supported scaffolds was not between 36 and 45 inches (note: crossbracing is acceptable in place of a midrail when the crossing point if between 20 and 30 inches or as a top rail when the crossing point is between 38 and 48 inches above the workplatform (see 1926(g)(4)(xv)):    a. At the construction worksite, 1406 Hwy 63S, Vienna, MO, employees worked on scaffolding, cross bracing was used as the guardrail without a top rail meeting the height requirement of the standard.
