29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Weiser Engineering/Mile High Precision Inc, at 10901 Irma Drive, Northglenn, CO: On and preceding 3/19/13, Weiser Engineering/Mile High Precision Inc did not ensure that protective equipment was used when necessary whenever hazards capable of causing injury and impairment were encountered.  Employees assisted the forklift operator with loading and unloading trucks in the paved area between the facility's two buildings.  Building lighting did not provide adequate lighting for the traffic area.  The employer did not ensure that protective equipment in the form of high visibilty clothing was used by employees who worked in the yard or in proximity to operation of the powered industrial truck.  This condition exposed employees to a struck-by hazard.

29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA 58-1969 "Storage and Handling of Liquefied Petroleum Gases":    (a) Weiser Engineering/Mile High Precision Inc, at 10901 Irma Drive, Northglenn, CO: On and preceding 3/19/13, Weiser Engineering/Mile High Precision Inc did not ensure that the storage and handling of liquefied petroleum gas fuel was in accordance with NFPA 58-1969 "Storage and Handling of Liquefied Petroleum Gases".  Employees operated a Hyster model 60 powered industrial truck while loading and unloading trucks in the paved area between the facility's two buildings.  The powered industrial truck was propane fueled.  Full and empty propane gas cylinders were stored inside the building that housed the shipping department.  The cylinders were adjacent to the building large overhead door which was used by forklift and pedestrian traffic.  The employer did not ensure that propane gas cylinders were stored in accordance with NFPA 58-1969.  This condition exposed employees to a fire or explosion hazard.
