Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. 1. In the warehouse, on the west wall, there was a 480 volt electrical panel box blocked by a large wooden A Frame.

29 CFR 1910.305(d): Switchboards and panelboards were not mounted in cabinets, cutout boxes, or enclosures approved for the purpose with a dead front: 1. Inside of the warehouse, in the northeast corner of the building, there was a 110/220 volt panel box with no cover in place over the electrical installation. There were live parts such as breakers and buss bars not enclosed to prevent arcing and employee contact with live parts.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles may have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 2.44 m (8.0 ft) above the floor may have exposed terminals. 1. Inside of the warehouse on the southeast wall of the building, near the dock, there was a broken (damaged) duplex outlet box. The receptacle(s) had receded back into the outlet box.
