29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.      a) 3181 Lincoln Highway E, Paradise PA 17562; Ladderjack Scaffold - Employees were observed working from a ladder jack scaffold 8 feet - 6 inches thru 22 feet - 3 inches above ground and were not provided safe access to the platform by the use of a portable ladder, on or about March 14, 2013.    No abatement certification or documentation required.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a) 3181 Lincoln Highway E, Paradise PA 17562; Upper Roof - Roofers were observed without fall protection while ripping off shingles from the roof of a residential home and were exposed to a fall hazard up to 22 foot - 3 inch fall hazard, on or about March 14, 2013.     b) 3181 Lincoln Highway E, Paradise PA 17562; Lower Roof - Roofers were observed without fall protection while walking across a lower roof to access a ladder jack scaffold and were exposed to a 12 foot - 10 inch fall hazard, on or about March 14, 2013.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) 3181 Lincoln Highway E, Paradise PA 17562; Lower Roof - Roofers were observed using a portable extension ladder to gain access to a roof and the ladder extended approximately 6 inches above the landing surface exposing the roofers to a 12 foot - 10 inch fall hazard, on or about March 14, 2013.
