29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets: This violation occurred on March 14, 2013, at a job site located at 1113 Navarro Drive, Allen, Texas 75013; where at least two employees were working at ground level in and around the perimeters of the site, exposed to falling and flying objects from employees working above.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: This violation occurred on March 14, 2013, at a job site located at 1113 Navarro Road, Allen, Texas 75013; where one employee was framing while working on the second level rafters, exposed to falling to the ground level below.
