29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Storage Yard, 48 Stonington Road, Mystic, CT. - On or about 3-15-13 - The employee was exposed to a crushed by hazard while in the process of changing a leaking high pressure hydraulic hose on a New Holland 16 LA bucket attachment that was not properly positioned and supported. The employee did not follow the procedure for supporting the attachment arm prior to releasing hydraulic pressure to the hose being replaced. The attachment arm dropped and pinned and crushed the employees leg between the arm and the front framework of the tractor.
