29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  At the job site,  2525 Bellerive Dr, Manhattan, Kansas, emplyoee operating the Caterpillar forklift had not completed formal training and evaluation program.  Employees were exposed to fall hazards of approximately 20 feet to the ground and stuck by hazards.

29 CFR 1926.451(g)(1):   Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the job site,  2525 Bellerive Dr, Manhattan, Kansas, emplyoees were working from a raised platform attached to a forklift and no fall protection system was provided.  Employees were installing stone work to the exterior column and exposed to fall hazards of approximately 20 feet to the ground.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  At the job site,  2525 Bellerive Dr, Manhattan, Kansas, emplyoees were working from a raised platform attached to a forklift and were not trained to recognize hazards associated with working from a raised platform and to impement procedures to control and minimize those hazards.  Employees were exposed to fall hazards of approximately 20 feet to the ground.
