29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. a. On or about March 14, 2013, Practice Building, A ladder or other adequate means of scaffold access was not provided and three employees climbed scaffold end frames to access scaffold platforms which exposes them to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about 3/14/13, Practice Facility Building, The ends of the scaffold outrigger platform did not have guardrails and employees who worked from the outrigger platform were exposed to falls greater than 10 feet to the ground.
