29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:    (a) On or about 06/28/2013 - The employer did not provide abatement certification for Citation 1, Item 1, by ensuring that employees were not climbing a scaffolds frame.   (b) On or about 06/28/2013 - The employer did not provide abatement certification for Citation 1, Item 2, by ensuring that employees working on a scaffold were protected from falling over 12 feet.  (c) On or about 06/28/2013 - The employer did not provide abatement certification for Citation 2, Item 1, by ensuring that a hazard communication program was developed and implemented for employees working with chemicals such as COOSA Buff Masonry cement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  (a) On or about 3/15/13, 101 15th Street, Tuscaloosa, AL, employees were exposed to a fall of over 12 feet while climbing the scaffold frame.
