29 CFR 1910.334(a)(2)(ii): Where there was a defect or evidence of damage that might expose an employee to injury, the defective or damaged item was not removed from service until repairs and tests necessary to render the equipment safe had made:  a) On or about March 14, 2013, and at times prior, employees were exposed to electrical hazards where the outer insulation on the cord for the "nippler" was damaged.
