TURK MASTERS CONSTRUCTION LLC WAS PREVIOUSLY CITED FOR A VIOLATION  OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT  STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313886483,  CITATION NO. 1, ITEM NO. 2, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON  12/23/2009, WITH RESPECT TO A WORKPLACE LOCATED AT 565 ELMWOOD  AVENUE, BUFFALO, NY 14222.      TURK MASTERS CONSTRUCTION LLC WAS PREVIOUSLY CITED FOR A VIOLATION  OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT  STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314790056,  CITATION NO. 1, ITEM NO. 1, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON  10/08/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 487 ELMWOOD  AVENUE, BUFFALO, NY 14222.      TURK MASTERS CONSTRUCTION LLC WAS PREVIOUSLY CITED FOR A VIOLATION  OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT  STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 577339,  CITATION NO. 1, ITEM NO. 2, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON  09/25/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 143 LAKE STREET,  HAMBURG, NY 14075.      29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 03/15/13, on the right side (when looking at the front) of house at 541 Crescent Ave.,Buffalo, NY:  Employee was exposed to a fall of approx. 15 ft. while walking and standing on the narrow aluminum pic of a ladder jack scaffold and assisting employees installing a new roof. No personal fall arrest system or other fall protection system was utilized.     NO ABATEMENT CERTIFICATION REQUIRED

TURK MASTERS CONSTRUCTION LLC WAS PREVIOUSLY CITED FOR A VIOLATION  OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT  STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 577339,  CITATION NO. 1, ITEM NO. 3, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON  09/25/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 143 LAKE STREET,  HAMBURG, NY 14075.      29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 03/15/13, on the right side (when looking at the front) of house at 541 Crescent Ave.,Buffalo, NY:  Four employees engaged in roofing work on a steep roof were exposed to a fall of approx. 19 ft. because no allowable fall protection system was utilized. The roof pitch was estimated to be approx. 12 in 12.  A double row of slide guards, one row about 7 ft. above the other had been installed as fall protection, but this does not meet the OSHA fall protection requirements of the standard. Additionally, all four of the employees were wearing full body harnesses, but they were not tied off to an anchorage point.      NO ABATEMENT CERTIFICATION REQUIRED
