29 CFR 1910.132(e):  Defective or damaged personal protective equipment was permitted to be used:    a)  In the Draw Down Room, on or about March 12, 2013 - Employees continually used gloves that were degraded and damaged due to being soaked through with paint solvents such as but not limited to Valspar Satin Clear which contained acetone, isobutyl acetate, propane, and butane.

29 CFR 1910.106(e)(2)(iii): Areas in which flammable or combustible liquids are transferred from one tank or container to another container were not separated from other operations in the building by adequate distance or by construction having adequate fire resistance:    a)  In the Production Area, on or about March 3, 2013 - The xylene was transferred from a 55 gallon drum to smaller metal containers and plastic buckets.  There were not any adequate separations from other operations such as storing flammables and transferring flammables in a booth from the floor xylene transfer station.

29 CFR 1910.106(e)(6)(ii):  Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degrees Fahrenheit (37.8 degrees Celsius), were dispensed into containers unless the nozzle and container are electrically interconnected:  a)  In the Production area, on or about March 3, 2013 - KS Xylol Xylene, a Category 3 flammable with a flashpoint of 81 degrees Fahrenheit, was dispensed from a 55 gallon drum to smaller metal containers and plastic buckets without electrically interconnecting the nozzle and container.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:    a)  In the applied sample assembly area, on or about March 12, 2013 - Employees were required to use elastomeric tight fitting respirators equipped with cartridges during transferring of organic solvents.  The employee's respirator was dirty and had not been cleaned prior to storage.    The employer, Vision Products, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(h)(i), which was contained in OSHA inspection number 314105198, citation number 1, item number 2e and was affirmed as a final order on March 17, 2010, with respect to a workplace located at Building 201, Schreiber Industrial Park, New Kensington, PA 15068.
