29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Worksite: The anchoring roof bracket that was used to anchor an employees personal fall arrest system was not attached to the roof with the required screws or amount of nails to ensure a 5000 pound capacity. Lecla, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.502(d)(15)(ii) which was contained in OSHA inspection number 316075613, citation number 1, item number 2a, issued on 1/31/2012 with respect to a workplace located at 61 Newtown Rd., Danbury, CT and became a final order on 5/4/2012.
