Second Level: Employees walking/working in the area were exposed to three floor openings that measured 8 inches in diameter that were not covered as observed on March 15, 2013. b. First Level: Employees walking/working in the area were exposed to approximately 19 floor holes that were approximately 2 feet x 2 feet x 6-8 inches deep located around the structural steel columns as observed on March 15, 2013.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Rooftop of Structure / Area B Column Lines 12 to 14: Employees installing metal decking were exposed to falls from heights of 16 feet 1.5 inches to 17 feet 10 inches from the roof to the floor below, and 33 feet from the roof to the ground below as determined on March 15, 2013. Abatement Certification Required For This Item.
