29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite: The employees who were working from the ladder jack scaffold platforms at heights of approximately 18-22 and 18- 24 feet above the ground were not protected from fall hazards by the use of guardrails or personal fall arrest systems.

29 CFR 1926.454(a)(1):   The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  Jobsite:  The employees who were working from the ladder jack scaffold without fall protection at heights of approximately 18 -22 and 18-24 feet above the ground were not trained to recognize the hazards associated with the type of scaffold being used.

29 CFR 1926.501(b)(11):   Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Jobsite:  The employees who were working on the 10 in 12 pitch roof were not protected from fall hazards by the use guardrails, safety nets or personal fall arrest systems.  Diaz Roofing LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 643458, citation number 1, item number 1a and was affirmed as a final order on 1/22/2013, with respect to a workplace located at 575 Boston Post Road Orange CT 06477.
