29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about March 18, 2013, employees performing utility work activities were working inside a trench and did not have a ladder for a means of egress. Employees climbed in and out of the trench using the trench side wall. Employees were exposed to entrapment and cave-in hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided. On or about March 18, 2013, and at times prior thereto, employees performing utility work activities inside a trench were not protected from falling rock/dirt and the spoil pile which was located at the edge of the trench on both sides. Employees were exposed to struck by and cave-in hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about March 18, 2013, Employees performing utility activities were inside a trench measuring approximately 11 feet deep, 14 feet wide and 43 feet long that did not have any protective systems such as but not limited to benching, sloping, or shoring at the last 20 feet of trench. Employees were exposed to being struck by and cave-in hazards.
