29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about March 15, 2013, at 211 White Road, Byron, Georgia:    An employee performing roofing work from the ground level was exposed to struck-by hazards of falling tools, roofing nails, and asphalt shingles from the employees working on the roof above him when he did not use a hard hat.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about March 15, 2013, at 211 White Road, Byron, Georgia:    Three employees performing roofing work were exposed to a fall hazard of approximately 11.4-feet to 14.4-feet while loading and installing shingles on the roof when they did not use fall protection.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):    On or about March 15, 2013, at 211 White Road, Byron, Georgia:    Three employees performing roofing work on a house were exposed to fall hazards that ranged from approximatley 11.4-feet to 14.4-feet when the employer did not train the employees how to recognize and minimize fall hazards.
