29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.   (a) Grinding Area: Employees tooled components, which created dusts that contain hazardous chemicals including but not limited to Cobalt, Copper, Chromium, Lead, Molybdenum, Manganese, Nickel, Tungsten, and Vanadium; but there was no written Hazard Assessment, on or about March 18, 2013.

Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.   (a) Establishment: Employees tooled components, which created dusts that contain hazardous chemicals including but not limited to Cobalt, Copper, Chromium, Lead, Molybdenum, Manganese, Nickel, Tungsten, and Vanadium, but were not provided Barrier Cream, on or about March 18, 2013.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.   (a)Establishment: Employees tooled tungsten carbide, and other steel components using grinding machines, CNC Machines and Wire EDM Machines. These processes created hazardous dusts including but not limited to Tungsten, Chromium, Cobalt, Copper, and Nickel. Employees were provided filtering facepiece respirators for voluntary use, but were not provided Appendix D, on or about March 18, 2013.

29 CFR 1910.1200(e)(1):   Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    (a)Establishment: A written Hazard Communication Program was not developed, implemented or maintained for employees who tooled tungsten carbide, tool steel and other steel components using grinding machines, CNC Machines and Wire EDM Machines. These processes created hazardous dusts including but not limited to Tungsten, Chromium, Cobalt, Copper, and Nickel on or about March 18, 2013.

29 CFR 1910.1200(h)(1):   Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.   (a)Establishment: Employees tooled tungsten carbide, and other steel components using grinding machines, CNC Machines and Wire EDM Machines. These processes created hazardous dusts including but not limited to Tungsten, Chromium, Cobalt, Copper, and Nickel, but employees were not provided effective information and training on the hazards of the chemicals with which they worked, on or about March 18, 2013.
