(a) On March 18, 2013, at the exterior south end of the ABC Tire Facility, employees were exposed to serious fall hazards from open-sided floors and or platforms in the following instances:             (1)  Employees accessed the east tire shredder unit by walking up the conveyor belt.          (2)  Employees standing on top of the east tire shredder unit to perform maintenance on the cutter blades.         (3)  The catwalk on the adjacent west tire shredder unit was not fully protected on all open sides by a guardrail system. 29 CFR 1910.36(b)(1): Two exit routes. At least two exit routes was not available in the workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section. The exit routes must be located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route.    (a) On March 18, 2013, in the ABC Tire Facility, employees were exposed to serious injuries in that there were no properly constructed exit doors in the facility for egress in the event of an emergency. Employees had to use the overhead dock doors which had be manually raised and then either jump or use a ladder to the ground 41 inches below.
